Q3, Q4, Q6, Q7, Q9: If your main priority is hearing from a lot of students, I would stray away from having required open response questions. These questions can often scare away students from filing out the entire survey. I would recommend making these kinds of questions optional, so students who either do not want to reply or do not have an answer to reply with still fill out the rest of the survey. However, if you feel responses to these questions are vital, I would try and pick which of the open responses are most important to you. Then, you can still require these, but leave the rest optional.
Q3: I would rephrase: ‘From the question above^^ briefly explain what is best about those services selected that make them great.’ To: ‘For question 2, feel free to briefly explain what makes the services you selected better than the ones you did not.’ And: Make this question optional

Q10: I would rephrase: ‘On a scale of 1-10, please rate how useful a future University-wide Physical Therapy and/or Chiropractic care and consultation service would be and how often you or those you know would use it.’ To: ‘Please rate how useful a university-wide Physical Therapy and/or Chiropractic care and consultation service would be and how often you or those you know would use it.’ Why: You include the 1-10 selection as well as what each end of the spectrum means, so you do not need to include this in the question.
Q11: I would rephrase: ‘What health insurance do you have? If it is not the University Health Plan, please select what type of insurance it is and specify the name of the program in "other".’ To: ‘What health insurance provider do you have? If it is not the University Health Plan, please select what type of insurance it is and specify the name of the program in "other". If you do not know the name of your health insurance provider, please write “I do not know”.’ Why: This allows students who are not aware of the name of their health insurance plan to still complete the survey. Otherwise, students may close out of the survey without trying to find the name of their insurance provider to avoid taking up more time.
